Abstract

A revolution in American medicine is in full swing, with the race from fee-for-service to fee-for-value at the front line in an epic battle that will transform healthcare delivery for decades to come. In America's Healthcare Transformation, eminent physician leader Robert A. Phillips brings together key thought leaders and trail-blazing practitioners, who provide a wide-ranging exploration of the strategies, innovations, and paradigm shifts that are driving this healthcare transformation. The contributors offer a panoramic look at the dramatic changes happening in the field of medicine, changes that put the patient at the heart of the process. Among other subjects, the essays evaluate innovative high quality and low cost care delivery solutions from around the United States and abroad, describe fundamental approaches to measuring the safety of care and the impact that guidelines have on improving quality of care and outcomes, and make a strong case that insurance reform will fundamentally and irreversibly drive delivery reform. In addition, America's Healthcare Transformation reviews the role of health information technology in creating safer healthcare, provides a primer on the development of a culture of safety, and highlights ground-breaking new ways to train providers in patient safety and quality. Finally, the book looks at reports from Stanford Health Care and Houston Methodist which outline how successful behaviorally based strategies, anchored in values, can energize and empower employees to deliver a superior patient experience. Drawing on the wisdom and vision of today's leading healthcare innovators, America's Healthcare Transformation provides a roadmap to the future of American healthcare. This book is essential reading for all health care providers, health care administrators, and health policy professionals, and it will be an invaluable resource in the effort to improve the practice of medicine and the delivery of healthcare in our communities and nation.

Abstract

Designed as a textbook for the DNP curriculum and as a practical resource for more seasoned health professionals, this acclaimed text encompasses an interprofessional approach to translating evidence into nursing and health care practice that is useful for both clinical and nonclinical environments. The second edition presents six new chapters, three of which feature additional approaches for translating evidence into practice, new methods of information technology for translation, and interprofessional collaboration and teamwork for translation. The three additional chapters offer 18 exemplars that illustrate actual translation work within the areas of population health and specialty practice, and in the health care system. Consistently woven throughout are the themes of integration and application of knowledge into practice, leadership and evaluating change, leadership strategies for translation, and interprofessional applications across settings. Also included is new information about outcomes management for improvement of direct and indirect care. The second edition continues to deliver applicable theory and strategies to achieve improved outcomes, and meets the DNP core competency requirements. It features a variety of models for change as they relate to translation of research into practice. The text underscores the importance of translating evidence for use in practice to improve health care and health care delivery, and presents strategies to achieve this. It addresses the use of evidence to improve nursing education, discusses how to reduce the divide between researchers and policy makers, and presents expedients for overcoming resistance to change. Extensive lists of references, web links, and other resources enhance learning and support the development of the DNP core competencies. -- Provided by publisher.

Abstract

"Health IT is a major field of investment in support of healthcare delivery, but patients and professionals tend to have systems imposed upon them by organizational policy or as a result of even higher policy decision. And, while many health IT systems are efficient and welcomed by their users, and are essential to modern healthcare, this is not the case for all. Unfortunately, some systems cause user frustration and result in inefficiency in use, and a few are known to have inconvenienced patients or even caused harm, including the occasional death. This book seeks to answer the need for better understanding of the importance of robust evidence to support health IT and to optimize investment in it; to give insight into health IT evidence and evaluation as its primary source; and to promote health informatics as an underpinning science demonstrating the same ethical rigour and proof of net benefit as is expected of other applied health technologies. The book is divided into three parts: the context and importance of evidence-based health informatics; methodological considerations of health IT evaluation as the source of evidence; and ensuring the relevance and application of evidence. A number of cross cutting themes emerge in each of these sections. This book seeks to inform the reader on the wide range of knowledge available, and the appropriateness of its use according to the circumstances. It is aimed at a wide readership and will be of interest to health policymakers, clinicians, health informaticians, the academic health informatics community, members of patient and policy organisations, and members of the vendor industry."

Abstract

Like the original Ward-based Critical Care, this second edition aims to guide practitioners in the initial management of critically ill adult patients within the ward setting. It should be of value to a wide range of healthcare professionals, including junior doctors, nurses, physiotherapists and students. It will also be relevant to practitioners who are moving into any setting that cares for acutely ill patients. This volume has been slightly reorganised since the first edition to ensure that practical aspects are incorporated within the relevant sections. The book is still structured around the ABCDE approach to assessing and managing patients, as this is promoted in critical care and resuscitation care training. The focus on practical examples and 'hot tips' to aid memory of key points continues. All the chapters have been updated with the most recent guidance, as available at the time of writing. This has meant that some chapters have required major revisions, while others have remained relatively unchanged. Some new chapters have been included, such as the ones on anaphylactic shock, patient safety, dealing with pregnant women in the ward setting, and massive blood transfusion. In addition, each section now starts with a chapter giving an overview of the A-E approach in order to provide more detail about assessing and managing those aspects of patient care. This informative book contains many useful features such as clinical scenarios, hot tips and practical skills, and the text is richly illustrated and cross-referenced throughout.
